Primary Care Wellbeing service
About the service
The Primary Care Wellbeing service is a team of specialist health professionals who work with people experiencing complex physical and mental health problems that significantly impact on mobility, mood and everyday living. This includes persistent physical symptoms, Post-Covid Syndrome (Long Covid) and long-term conditions.
We work flexibly and holistically, understanding the importance of links and connections between the mind and body and ensuring support is tailored to your needs, to enable you to achieve your goals.
We work closely with the other health professionals involved in your care, including your GP, actively liaising with them to provide you with consistent, wrap-around care.
We value improving the quality of people’s lives, supporting our patients to believe in their ability to succeed and improve their wellbeing, using a holistic approach which considers mental, physical, social and environmental factors.

How to find us
We may see you at home, at the GP surgery, at a health centre or remotely. This requires a commitment from both you and the team to ensure the best possible results.

Our team
We are a multi-disciplinary team made up of lots of different professions, including:
- Clinical psychologists – trained to work with people of all ages, with physical, mental and cognitive problems. We draw on research, neuropsychology and psychological therapies to assess and treat you.
- Assistant psychologist – works closely with the qualified clinical psychologist and other team members to support you to achieve your goals, help the team to monitor symptoms and treatment and to plan further care.
- Counsellors – working with you in a therapeutic, supportive, non-judgemental way to offer a safe space to explore difficulties and struggles and disclose information in confidence.
- Dietetics – working with you to help improve your health and wellbeing through nutrition.
- Occupational therapists – supporting you to complete daily tasks that you may be struggling with by finding alternative ways to complete them and to increase your independence.
- Physiotherapists – supporting you to achieve your goals around restoring movement and improving your physical function so you can return to activities that are important to you. We assess your needs and work with you one-to-one or as part of a group session.
- GP with special interest in persistent physical symptoms and long-term conditions – having undergone further study and training, and with specific expertise in this area, the GP provides a detailed, in-depth review of your medical and psychological history and offers assessments and treatments in support of recovery.
- Personal support navigator – offering advice on issues such as benefits or housing, and enabling you to access appropriate services across the health and social care systems.
