Adult Community Mental Health Assessment team
About the service
We support adults aged 18–65 who are experiencing serious mental health difficulties by providing thorough assessments and guiding them to the most appropriate services for ongoing care.
We are a small team of nurses, social workers, prescribers and therapists who review all new referrals into adult community mental health services. Our role is to ensure you are connected with the service that best meets your needs.
Sometimes we may invite you to an assessment appointment. This helps us understand your situation and guide you to the right follow-up support.
How to access
Please visit your GP if you would like to access the service. We only accept referrals from other health professionals. We don’t accept self-referrals.
After we receive your referral:
- We’ll send you a text message.
- A letter with next steps will arrive in 2–3 weeks.
- If you need an assessment, we’ll send appointment details by text and letter.
- Assessments are usually done within 4–6 weeks.

How to find us
We have five locations across the district. Our aim is to bring the care you need closer to where you live. All bases have clinic spaces for appointments.
If appropriate to do so, we can also work in community settings or in your own home.

What to expect
What to expect/ your assessment
- A 30–60 minute conversation with a mental health worker, this may be a nurse or social worker.
- We’ll talk about:
- Your health and background.
- Any safety concerns.
- What you’d like to happen.
- Together, we’ll agree on a plan.
You can choose:
- In person, by phone, or video call.
- At one of our clinics or at home.
- Bring someone with you.
- Ask for an interpreter.
- Request a different date or time.
- Choose to see a man or woman.
After your assessment
- We’ll create a plan to support you.
- Your care may then be provided by:
- Mind or Cellar Trust or Inspired Neighbourhoods
- The local Community Mental Health Team (CMHT).
- Other community support organisations
- You’ll receive a letter explaining what happens next.

DIALOG Questionnaire
What is DIALOG+?
DIALOG is a short questionnaire allowing you to rate your quality of life and experience of your care.
You will be asked to score how you feel in 11 different areas of your life on a scale of 1 to 7. There are no wrong answers.
Your mental health team will use your answers to have a conversation about what’s important to you. This means you can work together to develop a plan based on the things that really matter to you.

Repeating the DIALOG questionnaire at different times can help you and your team understand how you are feeling over time. There are different reasons for completing DIALOG again during your care, for example:
At the beginning of your care
Completing a DIALOG at the start of your care helps the team to know you and what matters to you better. It can be a good way of knowing where you are starting from.
During your care
There are different reasons for completing DIALOG again during your care, for example:
- If you start or finish an intervention (such as psychology): your care team may suggest completing DIALOG to understand how you are feeling about your quality of life. This might help look at things that have changed during the treatment.
- If you have big life changes: if your situation changes during your care (such as getting a new job) it can be helpful to complete DIALOG to make sure that your care is focused on the things that are most important to you.
- If you haven’t completed a DIALOG for a while: we recommend that you complete DIALOG at least every 6 months so that you can see if there are any changes and to ensure that we are still working on the things that are important to you.
- If you are transferring to a new team: If your care is moving to a new team it is helpful for you to discuss the DIALOG questions to ensure that the new team has the best information about what is important to you.
- At the end of your care with the Trust: At the end of your care it is useful to complete a DIALOG to understand any differences from the start of your care. It helps the staff understand what has worked well and how they can improve.

If you want to see previous DIALOG scores, you can ask your mental health team to show them to you. You should be able to see the scores from all the times that you have completed the questionnaire.
