Your health records
Your health records
When you receive care or treatment from our Trust, information is recorded in your Health Record. Your Health Record is confidential. It contains important information such as your name, address, date of birth, medical history, treatment history, and clinical and care notes.
To help provide safe, effective care, we share your Health Record with healthcare professionals who are directly involved in your treatment. You can choose who your information is shared with. You can also decide not to share any information (‘opt out’).
Sharing information helps ensure that the people caring for you have access to the information they need to make informed decisions about your treatment. Find out more about how we share your records and the choices available to you.
Accessing useful information online
We’re improving how we communicate with you and making it easier and more convenient for you to access information about your care. To do this, we’ve introduced a secure patient platform called Patients Know Best (PKB).
Patients Know Best allows you to view letters from the Trust online. Over time, additional information and services will become available through PKB.
You may already use the NHS App to access information from other places that care for you, such as your GP practice. The good news is that the NHS App and PKB can link together, giving you more ways to access information about your care.

Requesting a copy of your records
You have the right to request a copy of the personal information that Bradford District Care Trust holds about you. This is known as a Subject Access Request. Find out more about your rights and how to make a request.
