About the service
The Primary Care Wellbeing Service is a psychology-led, interdisciplinary team of specialist health professionals. We work with adults who face complex physical and mental health challenges that impact mobility, mood, and everyday life.
Our goal is simple: to improve your quality of life through a holistic, joined-up approach that connects mind and body.
Who We Help
- Adults aged 18+
- People with multiple long-term conditions and persistent physical symptoms
- Those whose needs lead to frequent GP visits, hospital appointments, or emergency care
- Patients registered with GPs in Bradford District and Craven
What We Do
We provide flexible, tailored support that focuses on your goals. Our team works closely with your GP and other professionals to ensure consistent, wrap-around care — so you don’t fall through the gaps.
How to access
If you think you can benefit from working with this service, please ask your GP for a referral.
How to find us
We may see you at home, at the GP surgery, at a health centre or remotely. This requires a commitment from both you and the team to ensure the best possible results.

Our team
We are a multi-disciplinary team made up of lots of different professions, including:
- Clinical Psychology – supporting mental health and coping strategies
- Occupational Therapy – helping you manage daily activities
- Physiotherapy – improving movement and physical function
- Dietetic – supporting nutrition and wellbeing
- Personal Support Navigation – guiding you through services
- GP with Special Interest – offering expert medical input
- We also link to community services – connecting you with local support
