Palliative Care service
Description
Palliative care is the care given to people who have an advanced progressive illness for which a cure is not possible. The aim is to identify and meet the needs of patients and their carers to help them live as well as possible.
Palliative care and support is provided by all professionals – GPs, hospital doctors, nurses and other healthcare professionals, for many people without the need for specialist palliative care support. The District Nursing Service is committed to supporting people with palliative care needs and should be involved prior to referral to the specialist team. They remain the key workers and coordinators of patients’ care needs continuing to provide nursing care when needed, at times working in conjunction with members of the Palliative Care Support Team.
The Community Specialist Palliative Care team may be asked to be involved in someone’s care to support the professionals already involved. This can include telephone advice/support as well as seeing people in their own homes, care home or community hospital.
The Community Specialist Palliative Care team consists of healthcare professionals including specialist consultant, clinical nurse specialists, liaison workers for south Asian patients. The team can also refer to other services including psychology and counselling and welfare advisors.
Who is the Palliative Care Service for?
We provide care for patients with a life limiting condition that needs specialist input. Patients must be over the age of 18 and registered with a GP in Bradford.
We provide care for patients in their own home across Bradford, Airedale and Wharfedale.

We welcome referrals from a health professional such as district nurses and GPs. We also accept referrals from hospitals and hospices.
