Health trainer service

Description

Are you ready to achieve goals to improve your health and wellbeing…? We’re here to help!

Our health trainers support and coach people to set and achieve SMART goals that can help improve physical, mental and social health and wellbeing.

The service targets disadvantaged communities offering free support and aims to address any inequalities.

If you are ready to make a positive lifestyle or behaviour change, our health trainers are on hand to support and encourage you. They can signpost you to other services and even accompany you to a new group.

Who is this service for?

If you are 16 and over, living in Bradford and Airedale, the team can support you.

How can you access this service?

To receive the help and support of our team you can self-refer, this means that you can contact the team direct by phone 01274 322666 or email healthtrainers@bdct.nhs.uk

Your GP or any other health professional may suggest that our health trainers can help you. In this case they can make a referral on your behalf using the online referral form on ‘SystmOne’ (a clinical record) or by calling 01274 322 666 .

Where can you see a health trainer?

You can see a health trainer in a variety of places including some GP practices, health centres, community centres, children’s centres, town halls and other community based venues. Call the health trainer helpdesk on 01274 322 666  for further information and to find out where your nearest health trainer is based.

Our health trainer office is open Monday to Friday 9.00am – 5.00pm. Our health trainers can work outside of these hours, by appointment only, this can be discussed when you call.

These free weight loss groups run for 12 weeks and will cover topics including food groups and nutrition, portion sizes, tips and food swaps to make favourite meals healthier. They are open to anyone aged over 18 who lives in the Bradford district. To join a group, call 01274 322666, or email healthtrainers@bdct.nhs.uk.
