Community matrons will help to co-ordinate and plan your clinical care needs and reduce any preventable admissions to hospital. They will undertake an assessment focusing on your health care history, carry out physical examinations and develop a care plan based on your individual needs.
If you require services from a specialist, our community matrons can make relevant referrals. They can act as an advocate for you making sure your views are heard and support you to undertake self-care techniques.
If your conditions get worse community matrons will closely monitor you and offer you the support you need based on a treatment plan with your GP to prevent admission to hospital.

Who is this service for?
Our community matrons support people in Bradford, Airedale and Wharfedale who are over 65 years of age, who have one or more long term conditions and complex needs and:
- have had admissions to hospital or who are identified as being at risk of admission to hospital
- high attendance at A&E
- can be identified by a GP or other health/social care professional to potentially benefit from further case management and clinical management/ monitoring input.
How can you access this service?
Your GP or any other health care professional may suggest that our community matron can help you. In this case they can make a referral on your behalf.
