Case managers

Description

Case managers understand how complicated it can be when you have a range of complex health and social care needs. They will help you and your family to manage your long-term health needs and treatment more effectively.

Case managers will help to co-ordinate and plan your health care needs and attempt to reduce preventable admissions to hospital. They will undertake an assessment focusing on your health care history and develop a care plan based on your individual requirements.

If you require a specialist service, case managers can make relevant referrals. They can make sure your views are heard and support you to remain independent, and in your own home, as long as possible, if that is your wish.

Our case managers are based at eight GP practices. You can contact your case manager by calling the GP Practice direct, or you can call 01274 256 131 .

Who is the Case Manager service for?

Our case managers support people over 18 years of age, who have one or more long term conditions, complex needs and have:

  • had admissions to hospital, or who are identified as being at risk of admission to hospital,
  • a high attendance at A&E, or
  • been identified by a GP or other health/social care professional to potentially benefit from case management.

To receive the help and support of our case managers you can self-refer, this means that you, or your family, can contact the team direct 01274 256131. 

Your GP or any other health and social care professional may suggest that our case managers can help you. In this case they can make a referral on your behalf.
