We support supports people that are discharged from hospital with medication. The nurses in our outpatients department regularly monitor patients physical health to make sure that their medication is still working for you. The types of checks include:
Blood tests
Blood pressure
ECG (heart check)
General wellbeing
How to access
We offer a drop in service and are here to support you if you have been discharged from our inpatient wards.
We are open Monday to Thursday
9.30am – 12.30pm and 1.30pm -4.30pm
