If you have recently received services from the Trust and would like to tell us about your experience there are two options for you to do so below.
Option 1 is for if you were provided with the services code at the time of your care and option 2 is if you weren’t provided with a code, you can select the service from a drop down menu instead.
If a staff member gave you a code to use to give us your feedback at the time of your care then please leave us your feedback here. You will be able to enter the code and then tick the boxes for each question and give your comments.
If you don’t have a code then please leave us your feedback here and you will be asked to pick from a list which service you are giving your feedback for, for example ‘district nurses’ or ‘health visitor’ you will choose and then tick the boxes for each questions and give your comments.
