Option 1- I was given a service code at the time of my care / appointment

– select the correct form;
– enter the code to fill your comments in; and
– Once finished, press ‘Complete Survey’.

 

 

Option 2- I was not given a service code

– Please click on the image on the left;
– select the correct form;
– press continue on the first screen;
– select the service from the dropdown list and fill in your comments; and
– once finished, press ‘Complete Survey’
