Learnings from independent investigation
NHS England commissioned an independent investigation following the homicide in 2017 of an individual related to mental healthcare, to identify areas for improvement. NHS England is now working with all the partner organisations involved to ensure that the review’s recommendations are actioned and shared with other relevant organisations.
Our Trust takes incidents such as this extremely seriously and whilst the patient was not in our care at that time, we took immediate actions to address learnings from both the internal and external investigations, to ensure that lasting improvements were made.
