In line with guidance from the National Quality Board (NQB) Learning from Deaths (LfD) Framework, we publish information on deaths and learning from mortality reviews on a quarterly basis.  The National Framework aims to ensure that there is a consistent approach to learning from deaths (both expected due to a clinical condition and unexpected deaths) across the NHS, and a transparent learning culture and commitment to continuous quality improvement, particularly for the care of vulnerable people.

The quarterly reports provide evidence that learning is firmly embedded across our Trust.  In line with the LfD Framework, information is anonymised and there is also a summary of thematic learning from the completed reviews.
