The term Care Programme Approach (CPA) describes the way used to assess, plan, review, and co-ordinate the range of treatment, care and support you can expect when accessing our specialist mental health services.
The principles of the Care Programme Approach are that:
- You are put at the centre of decisions about your care.
- We will work closely with you to develop a plan that promotes your social inclusion and recovery.
- Care assessment and planning sees you `in the round’, and supports you in all your roles and needs. For example, family life, creative interests and employment.
- Self-care is promoted and supported wherever possible.
- The needs of carers are recognised and supported.
- Services are organised and delivered in partnership with you, your family and carers where appropriate.
- Care planning is made possible through trust, team work and commitment. A care co-ordinator or lead professional should help to make this happen.
